I would say: Tsaw sleyku futa oe 'efu hiyìk nìtxan.

Not quite ...

Tsal (= tsa'ul) / Fì'ul sleyku/'eykefu oeti hiyìk nìtxan.
If 'it' is a thing.

Tsakemìl/fìkemìl sleyku/'eykefu oeti hiyìk nìtxan.
If 'it' is an action.

It is a thing, specifically a pain relief drug.

So, back again. I would really appreciate help wit the following .....

"I miss them so much."
"I miss you."
"It doesn't seem real."
"So much pain."
"I can't do this." [this action]

"Nìftxan pxaya tìsraw."

Quote"I can't do this."
"Tsa'uri oe ke tsun kem sivi."

Quote"I miss them so much."
Quote"I miss you."
I am not sure with these two :(

Usually, nga lom lu oer is "I miss you (=one person)"

Depending on who "them" is (2, 3, 4+), I'd say me/pxe/ayfo lom lu oer nìtxan

I would have said "ke tsun oe fìkem sivi".  Is this too direct for the purpose? I don't have the context.

The speaker feels unable to confront something (spiritually/intellectually rather than literally) that is incredibly painful (mentally rather than physically) for them.

Is this correct?

Ketuwong teng ke. Nì'eng ke. Aynga aoe keteng lu.

Humans aren't the same. Not on the same level. You and us are different,

Then I would say:
