EDIT: SO SORRY!!! I forgot to say my answers!

F/T/F.  I have had surgeries on all the listed things EXCEPT my foot: While I did sustain a fracture to my right foot, I didn't need surgery.  And I have been to another country besides the U.S.: Canada.
