I'd say:
Ngal kin srungit srak?
you-erg need help-acc (+marker of yes/no questions)
"Hrrap rä'ä si olo'ur smuktuä." ; "Ke'u ke lu ngay. Frakemit tung." (Assassin's Creed)

Well the word for help is srung, but it is a noun not a verb.  A verb to help someone would be srung si, as in "Oel poru srung soli" (I helped him).  But you aren't saying they are being helped, just ask if they need it.  The word for need is kin.  So asking if someone needs help becomes real simple.  Just state "You need help" and add srak to make it a question.

Nga-l srung-it kin srak?

(Damn my verbosity, it makes me a little slow to respond.)
Ftxey lu nga tokx ftxey lu nga tirea? Lu oe tìkeftxo.
